Umesh is right.The 12 volts is connected to an ignition coil (STEP UP TRANSFORMER)through a switch (CONTACT POINTS OR DISTRIBUTER) when the switch is INTERRUPTED a high voltage is created (ONLY ONCE FOR EACH INTERRUPTION) according to the design of the ignition coil.

Yes, Umesh and Pillay are right. The others are talking
about power boosting in electronics like a "blackberry",
NOT cars. Remember a sparkplug is a bolt of lightning
jumping an air gap (~0.6mm) with a huge resistance, which
is why you need 20kV.
Pillay's got it right, but for simplicity, picture charging
an inductor. Now you've got current flowing through it,
and if you disrupt that current (like open a switch) the
inductor doesn't like that, so it produces whatever voltage
is necessary to MAINTAIN the constant current. If you then
close a switch from that same inductor to your spark plug,
you've now got the voltage of the inductor placed directly
accross the spark plug air gap. Lightning.
