1.If there is loose connection between terminations of heating elements,hot spot will be made which leads to failure of heater.
2. If reaction takes place between heating element and surrounding atmosphere,i.e. corrosion of heating element which leads to poor heat transfer from heater to load and temperature of element becomes more than the maximum element temperature and heater fails.
