The phase angle difference between primary and secondary voltage in a transformer is always 180 degrees out of phase, not opposite in sign as you mentioned. However, the power factor can be improved by reducing the phase angle difference between the current and voltage, which means leading or lagging the current to match the voltage more closely. To improve the power factor on the primary side, the secondary side should lead (if the power factor is lagging) or lag (if the power factor is leading). The power factor improvement doesn't require changing the sign of the secondary voltage; instead, it involves adjusting the phase angle.
