Forebay is like open channel whereas surge tank is like close circuit. For forebay net head need to considered from forbay water level, whereas in surge tank, net head can be considered from dam height since it's pressure flow condition.

Forebay is put immediately after reservoir which is followed by gate,penstock,& surge tank respectively.Though basic function i.e regulation of pressure & velocity of water is the same, forebay is directly associated with reservoir & its water level; whereas surge tank is associated with gate & turbine load directly.Forebay prevents overflowing or drying of reservoir temporarily whereas surge tank continuously regulate pressure & velocity of water just before the entry in turbine thus preventing water hammerings.
