Whenever there is a fault in the system there will be a
momentary dip in voltage as breaker takes some time to clear
the fault. The momentary rise is due to sudden reduction of
load or lightning stroke

It is due to variation in loads. It also occurs due to fault in the network.

Actually voltage dip is the sudden decrease of rms voltage of transmission line. And voltage swell is the temporary increase in RMS value of AC voltage with the magnitude of the retained voltage between 110% and 180% of the rated voltage. These are mainly due to the fault occurring on any of the power system components.
