Condition Monitoring doesn't need to be with instrument, Visual, feel and measure are the 3modes of condition monitor techniques, it depends on the level which one to use and when.
Example, Say You are watching a Gear Box and found Lub oil level is ok and no abnormal heat, this is condition monitoring check with 1st and 2nd level. 3rd level with go with by finding the internal ingredients levels and compositions changed due to wear and tear, dust entries etc..., Similarly with bearings say you have touched the bearing and found no abnormal vibration and Temperature, if Temp is high or Vibration level is high felt, then measure what levels they are to know the permissibles and actions.
