CMM is no longer used. CMM was made obsolete in December
2006.
Only CMMi is used and the current version is CMMi ver 1.2.
The exact procedure obtaining a CMMi certification can be
obtained for the Adoption site of SEI.

The site which gives details about CMMi Adoption is as
follows:
http://www.sei.cmu.edu/cmmi/adoption/

First company need to register and start with SEI CMMI Level 1 and start following process mentioned and get project audited from  an authorized CMMI certified person
following process strictly company can reach level 5 after some yeare
