In my company, we are getting test case templets from Client
(AVIVA)

1.It basically depends upon the testing practice of a
company/Project.
2.If QC is used then it has its own simple format.
3.When the clients ask for a specific format as mentioned in
the above comment, no doubt it must be followed (provided if
they are deliverables)
