Test Plan is a running document, when a testing phase starts and the Test Manager is “controlling” the activities, the test plan should be updated to reflect any deviation from the original plan. After all, Planning and Control are continuous activities in the formal test process.

It's a running document. Because it is plan, it still not happens yet, it just expected result. So creator should update it during project time.

it depends, if team is done with the requirements and coding
test plan will not change... testing team is just to execute
it if development and requirements are yet to finish then
sricha comments may sense...
