Hi, send a mail to ETS informing the problem.
they consider one request without payment ( but since it is
more than 5 months they might ask you to pay $17)and send
you one more score report.
here are the details to get in touch with them.

Dear, One of my close friend is in B2 Visa. Twice she went in B2 and came back to india. First trip to US in June 2008 and second trip to US in Oct 2008. Hi all, my F-1 visa request was rejected two years ago (December 2008) and now I will try for the second and last time on January 7,2011. Previously my brother, who lives in the U.S.A, was my sponsor but this time I'm self sponsored and changed my university. Last time the VO said "You are going to work with your brother" and rejected my visa. So the possible questions for my visa will be as follows and please give me some possible answers. Thank you!
