Every human being makes lot of mistakes.that way every
software should have lot mistkes.mostly software mistakes
are the fallowing resons

mis communication or no communication
software complexity
programing errors
changing requirment's
time presser

This all phases are Requirent to be bugs free software.
1.Analysis phase
2.Design phase
3.Coading
4.Testing

As Per my exp. Programmer should perform Unit testing,
before he pass to Tester.

+ the discrepancy in the interpretation of the requirements
spec or functional spec.

+ The human errors at every phase of development

+ not following the software engineering process
