Yes, it is possible. It may occur while fixing previous
bugs. This is the main purpose of doing Regression testing
to find fixed bugs affected other parts of software

When it is possible to get high severity bugs when the
product is live why cant it be possible on the third round
of testing?
This question is foolish, if this is the only question but
very smart if the interviewer asked this question to get to
an argument with the candidate with an open mind to learn
the candidate perspective.
