I don't know the process in your company, which methodology
you follow - but the right thing to do is address the gap
to the document "author" in which document you find the
gap and request to update the document.
The main thing here is the "GAP" (which could be a missing
requirement, incorrect req, conflicting with other req)
should be addressed by the identified person only. since it
makes a lot of difference if you go through a layer process
to address it instead of addressing directly.

Very simple answer is hwen ever we feel that (Due to our
experience)funnctionality of the application is good but it
is deviate from BDD
or there is a differance in BDD and another requirement
document
or while client interactions whatever we catch is wrong
when compare to BDD then we confirm that BDD is wrong

When we find something wrong in BDD(Business Development
Document) ,it has to be reported immediately to the
reporting person and he will take it to the top level
management ,the top level mgt will discuss the issue and if
its really wrong then they interact with the client and
then rectify the mistake.
