Below are the fields u need to have while raise an issue:
Bug description, date, issue id, Bug summary, Bug
description,severiy level, priority level, reproducible
levels(always, random or only once), reproducible steps,
attachments(screen shots, videos) if require.

Different company will follow different template for test
cases, but in general we will have the following:
Test case ID, Test Description, Executable steps, Test data,
Expected result, Actual result, Test Result(Pass/Fail),
Environment(OS or BROWSER) specific

- Bug No ,Module Name/Sub Module Name ,Bug Location,Bug
Description,Owner of the Bug,Severity Level..etc.

Fields Present in Testcases are...

- Module Name,Prerequisites,Test case No,Action,Expected
Result and Actual Result,Pass/Fail Criteria.
