A positive Trendelenburg's sign indicates weakness or injury on the side opposite to the affected hip. In this case, a gunshot wound to the right buttock could cause such a condition due to damage to the right iliopsoas muscle (a hip flexor) or the right iliacus muscle. The patient would tend to compensate by tilting the unaffected side upwards when standing.
