Raising a defect is nothing but filing it in bug tracker
tool with assignment to the particular person who is
responsible for it for e.g. Dev Lead.
The things which we should follow before raising a defect;
1. First analyze whether its really a defect
2. Check its reproducibility on Build which is under test
3. Check its reproduciblity on previous build so that we can
get info like defect is regression or new defect.
4. Check whether it needs particular environment for its
reproducibility
5. Decide its severity based on impact on application.
6. Last but most important to have a proof for e.g.
