failing means if there is a bad sectors on disk , the I/O
will be fail. but there is no data loss. for example in
veritas if vxiod write option failes then it it shift the
private region to public region
failed means disk is totally failed to I/O operations. in
this case vxiod transfer the failed info to the vxrelocd.
vxrelocd uses the hot spare disk or wait for disk
replacement by the admin.
