Dear sir,
following are the procedure to be followed by a suppplier
if he lost the C form issued by the purchaser.
1- Intimate your sales tax office for the lost of C form.
2- intimet your purchaser and give the following document
for issueing Duplicate C form.
1- indemninty bond (Form G)of Rs. 200 avilabe at cst site
or frenking the Form G of Rs. 200.
2.Two advertisement copy one should of english and one in
Local laguage for lost of c form giving full discription of
C form.
3.FIR report of lost of C form.
4- request for issueing duplcate c form to your purchaser.
