THE MAIN POINT IS DUE TO EXCESSIVE HEAT, THE REASON MAY BE AS LOSS CONNECTION,CONTAMINATION OF TRF. OIL WITH MOISTURE,WITH NOT PROPER COOLING, AND EXCESSIVE LOAD.
