I am tackling this question as if your are the team lead.
1. Asses his strengths and weaknesses.
2. Asses his present condition is due to demotivation, lack of motivation or inadequate motivation?
3. Is he not able to fit into this team or there is a temporary slump in his Performance.
4. sit with him and counsel him about his performance or lack of it.
5. If after all this he does not come up in performance change his duties after ascertaining about his aptitude.
6. Counsel him as to how and why of his present inadequate performance.
