I have a single cycle maintenance plan that is triggered by
a counter assigned to a equipment A. If I replace equipment
A with equipment B how do I change my Maintenance plan
counter? When I use IP02 "change maintenance Plan" the
counter box is greyed out. Thanks for any advice.

Create a new counter and plan for equipment b

Do we need to run this transaction code ip30 for all the maintenance plans which are scheduled on regularly daily basis or weekly basis so that if any scheduled object exists then it will get converted into the service order ?
