What do you think is a good investment, a defect tracking
system or a test management system integrated with a defect

It depends on what goals you pursue.
My experience is its better to separately utilize systems
in order to get better clarity and results .
Having tried like a dozen of services my choice is BUGtrack
as defect tracking system.
