its answer depend on the project as mainly banking domain
project companies used waterfall model .but on small project
now a days mainly small companies are using incrimental model.

You must test a trouble report where the problem cannot be reproduced. You tried using various test data files and mutations from the data files the customer sent along with the trouble report. The software does not fail the way it did at the customer's site.
How can a LOG for testcases be maitained which can give information about a TestCase that is it new one or has been occured in previous versions of the software.
