Before PIN once the project enters into the company there will have a START UP meeting in that the people like project managers of diff domains,developer lead, test lead............all manager level people will be there.
some times client representative will be there in the meetings, in that the will select project manager
PIN stands for PROJECT INITIATION NOTE
will be prepared by the project manager first he will ask the CEO to start the project thru E- mail
and once he accepted the PM will sent E-mail to all of his core team members for conformation.
and then SDLC starts...........

If you are handling a product testing and if there are around 100 test cases and in them few test cases are failed then at the end of the day how will you treat whether the product is pass/fail.? consider the failed test cases are in functional requirements.
