ORA-16736: unable to find the destination entry of standby

Cause: Either the standby destination was manually changed
or deleted outside the Data Guard broker's control, or no
entry was available for Data Guard broker.
Action: Clean up destination setting, remove the unused
ones, and reset the log transport service.
