Incident - Product delivered to customer and he reported an 
issue in the application.

Defect - Issue found during testing phase by QA team and it 
was logged.

when a work product is delivered to a customer and during 
the maintenance pillar if any thing is found wrong that is 
registered as an incident

When a work product is delivereda nd during the test phase 
if any part goes wrong then it is a defect
