All these terms are inter-related
Error :any mistake due to human imperfection 
Defect: the error may cause a defect 
Failure: these defects may cause failure
Fault : is nothing but same as defect

error : introduce bug by human mistake
failure : mismatch between actual and expected is failure

Expected Output.
Failure is the inability of a system or component to 
perform the required function according to its 
specifications.
Fault is a condition that causes the software to a fail to 
perform its required function.
Defect is a coding error in a computer program.
