As far as my knowledge 

Report Defect means once you identify a defect you raise 
(report) the Defect and the status is NEW

Track Defect something you will track the defect status 
once it changes to Open, awaiting functionality, fixed, 
Ready for retestetc. you obsereve the change in status of 
that defect unitl it status changes to Close.

Diffect report
Whenever we found a deffect we report that diffect by 
giving some mandatory information about that diffect like
1). Diffect id
2).summary
2).screen short
4).description(detailed one)
5).Assigned to
6).Tested by 
7).seviority
8).priority
9).Release...etc
Where as deffect tracking
after reporting the deffect, the developers perform some 
actions like or stages in solving the problem(deffect)
it is also called deffect life cycle it inludes particulars 
like NEW, REJECTED, OPEN, DEFFERED, REOPEN, VERIFIED, 
CLOSED.

It is very simple to understand" we publish a report after 
we track the defects".so that process of understanding all 
the defects a process can have can be put in a defect 
tracker after which a consolidated POA will be published in 
a report.

TRACK DEFECT:
While testing the application i.e execution of test cases, 
if expectd value is deviated from actual value then we will 
set the status of the bug as NEW and send that buglist to 
developers. 
Then developer verifies whether it is really a bug or not. 
If he feels it as a bug he will set the status as OPEN and 
he starts rectification. Once rectification is completed 
and before sending it to the development dept he will set 
the status as FIXED.
Else
There are 3 cases
i. If he is in confused state i.e to accept it or not as a 
bug, he will set the status as HOLD.
ii. If he feels i.e not at all a bug then he wills et teh 
status as Rejected/ Testers Mistake.
iii. If the tester don't know about that functionality 
newly incorporated by Client then he will set the status as 
AS PER DESIGN.

Once the bugs are rectified the development dept will 
release the second buils to the testing dept then the 
testers will test the defect functioanlity as well as new 
functionality. If the defects are rectified then they will 
set the status as CLOSED. Else they will set the status as 
REOPEN. IF any new defects identified set the status as NEW 
and they will send the buglist to development dept
